ST. LOUIS—Optometry Cares—The AOA Foundation and the InfantSEE program are accepting nominations for the 2016 Dr. W David Sullins Jr. InfantSEE Award. The award is given in honor of Dr. W. David Sullins’ dedicated leadership as a driving force in the profession of optometry, an announcement said.

The winning optometrist will be awarded during this year’s Optometry’s Meeting, which will be held in Boston from June 29 to July 3.

Nominations must be submitted online by April 15, 2016.

Nomination forms must include:
• 1,500 word-or-less statement that details why the nominee should be considered for recognition and explain the nature of the individual’s public service/contribution for InfantSEE.
• Brief, type-written biographical sketch.
• Award recipient must submit a current head-and-shoulders photo.
